Flying Congested Areas

We are currently in the process of finalising our operations manual and sending of our SRG1320 form to the CAA. One thing we have always been curious about is flying in congested areas. We are totally confused!

The CAA state, you must not fly over or within 150 meters of a congested area. We have heard a bunch of different things regarding getting permission/exemptions to this.

So I ask you, what are we supposed to do to receive permission to fly in a some regard in a congested area?

For aerial video and photography a high percentage of jobs could be in congested areas, and so there are a lot of reasons why one might want to fly in that kind of area. Any help would be great.
